Case Manager - WARM

Salary Range: $13.39/hr - $15.15/hr DOE
Working Hours: 40 hrs week; Mon-Fri; 8am-5pm with flexibility

This is a temporary position lasting from Dec 4th, 2017 - February 17th, 2018
A currently competent Case Manager who will be responsible for providing professional care to women and/or their pre-school dependent children in a residential setting and follow-up with former residents in SMA supportive housing who are experiencing problems with substance abuse and may have additional diagnosis of mental health problems. The case manger will utilize; comprehensive case management services, documentation of services and needs, waiting lists/admission/ discharge /transition case planning, and implement effective communication and listening skills. Primary program liaison with service providers and agencies, maintain linkages with client and client's family, coordinate transportation as needed for required services, attend client treatment team and other related staffing. The Case Manager will serve as court liaison with Drug Court, Department of Corrections, Criminal and Civil courts, DCF, and other agencies as required. Case Manager will perform other related duties as required by the client, family, Program Director, and Stewart-Marchman-Act Behavioral Healthcare Services.
Qualifications
Graduation from an accredited school with a Bachelor's Degree; preferably in Psychology, Social Work, or some other relevant Human Services. Currently certified as an Addictions Professional in the State of Florida is preferred. Experience working with substance abusing population preferred. Current valid driver's license in the state of Florida is required. Current CPR/First Aid certificate required.
Must be knowledgeable and demonstrate competence in current principals and including connecting clients with community resources, discharge/transition planning, and familiarity of aftercare programs in Florida, which are clinically specific to substance abuse.
Must be familiar and demonstrate competency in effective case management techniques and behavioral and clinical management of clients.
Knowledgeable of typical substance abuse diagnoses.
Demonstrate familiarity with software programs used at SMA, or the ability to learn them in a reasonable period of time.
Demonstrates safe operation of computers and associated hardware.
Knowledgeable of HIPAA and State confidential regulation as they pertain to health information.
Understand DSM-IV substance abuse and mental health diagnosis.
Ability to make appropriate referrals for dual-diagnosed clients.
Responsibilities
Records services and outcomes, submits confidential reports as required.
Identifies proper forms for case management documentation and correct placement in client record.
Documentation of activities including outcomes, and plans, using DAP format.
Produce appropriate reports to judges, probation, child-family agencies and referral sources in a timely efficient manner.
Adheres to HIPAA, safety and security rules and procedures.
Implements policies in daily work assignments with staff, community providers, clients and their family.
Maintains professional communication and attitude with community providers, program staff, residents, their children and family members.
Coordinate with staff, community providers, and residents their children and family positive interpersonal skills to obtain resources and resolutions to client related needs and services.
Works with Project WARM interdisciplinary treatment team, community providers and partnerships to ensure best practices with client case planning and services.
Maintain on-going contact with courts, probation, Department of Children, Community Partnership for Children and other client linkages to maintain client outcomes.
Serve as a court liaison.
Contact made by telephone, in meetings, letters, and other paperwork important to client services.
Collaborate with psychiatrist/psychologist/ARNP and other treatment team members.
Develop treatment planning, on-going assessment, and discharge planning.
Case Manager will assist in coordination of performance plans, monitor client attendance, provide and refer clients to develop sober life skills in the areas of daily functioning, employment, education, and self-care needs.
Assist client and their children with transitional services for aftercare.
Maintain and manage Project WARM wait list and coordinate admissions.
Case Manager will evaluate potential clients and place on the program waiting list.
Will maintain the waiting list according to DCF, CARF, Project WARM and agency standards.
Will document contact with women on the wait list.
Will prioritize and coordinate wait list for admissions into the program.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
